List Players in Jersey Number order
In case a player is absent, line out the player's name and write in absent.
Complete bottom portion:  Date, Time, Field
Complete first line of reverse side

Referee Team:

Mark Goals on Player's Line
Mark Goalkeeper (G) in corresponding Quarter on Player's Line
Mark Player Out for the quarter (X)
Mark Half Time and Ending Scores Winning/Losing Teams at the Bottom of the Card
Complete SPORTSMANSHIP on Reverse Side and provide referee team names &
phone numbers in case of questions

After rating both teams, please print your name and phone number at the bottom and provide any comments
Place the completed game cards into the field box at the field.

Referee Team - 10U

10U REFEREE TEAM- Rate the team on the side you are working,
but provide comments to the other Referee Team member rating the team on the other side

10U REFEREE TEAM - X the box if you are rating your own game or your assigned game

Please print names and phone numbers of the Referee Team at the bottom
and provide any comments.

Place the completed game cards into the field box at the field.
